Social Media Consent Form

Purpose of Consent

We love sharing the special patients who visit our hospital! With your permission, we may take photographs or videos of your pet during their visit, treatment, recovery, or other hospital activities to share the wonderful work we do and celebrate our patients.

Client Privacy

I understand that:

• My pet’s name may be shared alongside photographs or videos.
• My name, contact information, and other identifying personal information will not be shared without my separate permission.
• The hospital will make reasonable efforts to protect my privacy when posting images or videos.
• Content shared on social media may be viewed, saved, or shared by others outside the hospital’s control.

Voluntary Consent and Withdrawal

I understand that granting permission is voluntary and will not affect the veterinary care my pet receives.

I may withdraw my consent for future use at any time by contacting the hospital in writing. The hospital will make reasonable efforts to stop future use of the images and remove content from its own accounts where practicable. I understand that previously shared content may have been copied, reposted, or saved by others and may not be fully recoverable.
